Prevención del reingreso hospitalario del paciente crónico adulto

Abstract

Durante las últimas décadas la población española ha sufrido un envejecimiento continuo. Este tema ha sido el punto de mira por muchos países, no solo por los reingresos hospitalarios sino también por la gran demanda sanitaria y un aumento del gasto económico que provoca. El objetivo de este trabajo es revisar las principales estrategias de prevención de reingresos hospitalarios en pacientes crónicos. Se realizaron búsquedas en las principales bases de datos, en las páginas web de salud y en revistas científicas. Se ha evaluado cada artículo teniendo en cuenta 3 variables para medir las estrategias: <<calidad de los cuidados sanitarios prestados por los profesionales», “efectividad del tratamiento» y <<cumplimiento adecuado o inadecuado de las pautas terapéuticas y el estilo de vida del paciente» Los datos fueron positivos, no obstante se discuten recomendaciones y se proponen nuevas líneas de investigación para encontrar nuevas estrategias o reforzar las ya encontradas.

During the last decades, the Spanish population lived a continuous aging. This subject has been the target of several countries, not only because of the readmissions but also because of the high healthcare demand and the rise of the budget expenses that it produces. The aim of this work is to check the main prevention strategies for the readmissions of chronical patients. Researches have been carried out on the main data basis, in the Internet thru health websites and scientist magazines. Each article have been graded regarding three variables in order to height the strategies: “Quality of healthcare handled by professionals", “Treatment efficiency" and ”respect -or not- of the therapeutic rules and way of life of the patient”. The data were positive, however recommendations are discussed and new investigation lines have been proposed in order to find new strategies or in order to improve those which already exist.
