Atención de enfermería y manejo intrahospitalario de pacientes con ictus en fase aguda

Abstract

El ictus representa una de las primeras causas de mortalidad en la población general a nivel mundial y supone la primera causa de discapacidad, siendo una de las principales causas de morbilidad en todo el mundo. Una atención precoz y el adecuado manejo intrahospitalario durante las primeras horas tras el ictus influirá en la evolución del paciente. El objetivo de este trabajo es revisar la literatura disponible acerca del manejo intrahospitalario del paciente con ictus en fase aguda. Para ello, se han consultado las siguientes bases de datos: Cuiden Plus, IME (Índice Médico Español), PubMed, LILACS y Biblioteca Cochrane Plus. Han sido considerados como registros útiles y a partir de los cuales se lleva a cabo nuestro trabajo 37 artículos que incluyen 12 revisiones sistemáticas, 23 artículos de revista, 1 informe de evaluación y 1 protocolo de actuación del Complejo Hospitalario de Jaén. Los resultados obtenidos manifiestan cuáles son las necesidades básicas que se alteran en una persona tras un ictus, las principales complicaciones que pueden presentarse en la fase aguda de la enfermedad así como las actuaciones de enfermería más adecuadas durante la hospitalización de estos pacientes en fase aguda. También recogen los beneficios que se obtienen con la atención en unidades de ictus. Podemos concluir que la atención temprana y especializada en unidades de ictus en las primeras horas tras el ictus reduce la mortalidad y el daño cerebral, y por consiguiente, la dependencia y aumenta la recuperación funcional de estos pacientes.

Stroke represents one of the first cause of mortality in the general population worldwide and it supposes the first cause of discapacity, being one the main causes of morbidity over the world. An early care and the proper hospital management for the first hours after the stroke will influence in evolution patient. The object of this project is to revise the literature available about the hospital management of patient with stroke in acute phase. For that, the next databases have been consulting: Cuiden Plus, IME (Spanish Medical Index), PubMed, LILACS and Cochrane Plus Library. They have been considered as useful registers and from which it takes just our work, 37 articles including 12 systematic reviews, 23 magazine articles, 1 evaluation report and 1 protocol for the Jaen Hospital. The results obtained show what are the basic needs which are altered in a person after a stroke, the main complications that may appear in the acute phase of the disease as well as the most suitable nursery actions for the hospitalization of these patients in acute phase. They also collect the benefits obtained with care in stroke units. We can conclude that early and specialized care in stroke units in the first hours after the stroke reduces mortality and brain damage, and therefore, the dependence, and it increases functional recovery of these patients.
